The membership application form is in PDF format. Page 1 must be printed, filled out and mailed to the CCA office (address is on the form) with payment. Page 2 is to be kept for your reference.

Membership runs for twelve months. The membership year for members joining after October 1, 2006, runs from the last day of the month you join, until the same date the following year. Memberships continue to expire on December 31 for people who were members prior to October 1, 2006. Renewal forms will be sent out from the national office in advance of the renewal date.
